According to Dave Meltzer, Triple H and Vince McMahon were both absent from TLC this past Sunday. It's said that they were conferenced in on the meeting before the show and the PPV was put together just like McMahon wanted. However, it was Paul Heyman in the Gorilla position producing the announcers. 

He was also in charge of RAW's matches, while Bruce Pritchard took control of those for SmackDown

Meltzer notes that the feeling backstage is that the show was handled poorly and that what happened with Kairi Sane was an indication of that. As has already been pointed out, a doctor should have been told to pull her from the match rather than allowing her to continue taking bumps which could have worsened her possible concussion (she's still not cleared to wrestle).
